One Charlotte Coordinated Entry / Gulf Coast Partnership

The One Charlotte Homeless Services Team helps residents navigate the path to housing by offering tailored support every step of the way. We’re committed to ending homelessness in Charlotte County, one individual at a time. Our centralized Coordinated Entry process is designed to streamline access to housing and support services for Charlotte County residents experiencing homelessness or at imminent risk of becoming homeless in 14 days or less. This process ensures that each individual or family is assessed, prioritized, and connected to the most appropriate housing interventions based on their unique needs and circumstances. To access services, Charlotte County residents can visit One Charlotte’s Coordinated Entry Access Points located throughout the county; or call our Coordinated Entry Hotline; or complete the One Charlotte Coordinated Entry On-line Access Form at https://docs.google.com/forms/d/e/1FAIpQLScbxPQMsa7CtmqZ7TrwXOOGQupSxxnGJ3DY8wm7MLjqIlqZ_w/viewform
